INFORMATION WE COLLECT

As part of our commitment to provide the highest quality financial products and services, Provident collects non-personal information about you from the following sources:

  • Applications, forms, and other information that you provide us,whether in writing, in person, by telephone, electronically, via the Internet or by any other means. This may include your name and address, as well as, employment, income, assets, and credit information;
  • Information that we receive when you visit or transact business on our web site, by means of an information-collecting devise known as an Internet “cookie” and other online tracking technologies;
  • Your transactions with us, our affiliates, or others. This information may include your account balances, payment history, and account usage (including ATM and/or Debit cards);
  • Consumer reporting agencies. This information may include account information and/or information about your credit worthiness and credit history; and
  • Other third parties, including public sources. This information may include real-estate records, motor vehicle reports, demographics, street and e-mail addresses and telephone numbers.

The Provident Bank complies with Section 326 of the USA PATRIOT Act. This law requires that we collect and verify identification information from our customers at the time a new account is opened. We collect only that information necessary to deliver quality products and services to our customers or as required by law.

INFORMATION WE SHARE

We are permitted by law to share information we have about our experiences or transactions with you or your accounts (i.e. account balances or payment history) or collected information (i.e. credit reports or assets and income information) with our affiliates.

We also are permitted by law to share non-public information about you to companies that perform marketing services on our behalf or to other financial institutions with which we have a joint marketing agreement. In addition,we may provide all the information we gather about you to our service providers. These providers may include check processing, statement rendering, computer operations, and website management companies. All contracts with joint marketers and service providers include a provision that the contracting entities agree to protect the non-public personal information of our customers and that use of this information is restricted to that purpose for which the contract has been agreed to.

We do not otherwise sell, rent or provide customer information to outside companies (including those companies offering mortgage or other credit insurances) so they can market their products or services to you.

If you do not wish us to share your information with Provident affiliated companies, nonaffiliated third parties or joint-marketers, other than that permitted by law, you may exercise your right to opt out of this activity. To opt out, please call us toll-free at 1-800-448-PROV (7768) or write to us at the address listed on the back panel. When writing to us, please make sure that your request clearly indicates that you do not wish to have your collected information shared and that your correspondence clearly identifies you, your Social Security number and your affected account numbers. If you have previously informed us of your preference to opt out, you do not need to do so again. We will honor your choice of restricting information sharing with Provident’s affiliates. However, this restriction does not apply to your account transactions and experiences with us, nor does it apply to our identification information verification process.

If there are multiple account or product owners,we will treat the opt out directive by a joint owner as applying to all of the associated joint owners. However, should any joint owners have other accounts with us independent of you, your request will not apply to those other accounts.

We are required or permitted by law to disclose non-public personal information about our customers in the following circumstances:

  • To respond to subpoenas and other legal processes, regulatory authorities or law enforcement agencies;
  • To consumer reporting agencies;
  • In connection with a proposed or actual sale, merger, or transfer of all or a portion of a business unit;
  • To protect against fraud,money laundering and other risks; and
  • To those with whom you have requested us to share information.
PROTECTING CUSTOMER INFORMATION

Access to your personal information is limited to those employees who need to know the information to provide products and services to you. We maintain physical, electronic and procedural safeguards that comply with federal regulation to protect your non-public information. Our employees regularly receive training as to our privacy policy and practices.

What you can do to protect your financial information:
  • Routinely review and reconcile your account statements. If you become aware of incorrect information, either in our records or as reported by us, please notify us by calling 1-800-448-PROV (7768). We will promptly research the discrepancy and make the necessary corrections.
  • Safeguard and protect your account records, statements, access devices and other information pertaining to your relationship with The Provident Bank.
  • Never provide your account number, personal identification number (PIN), or password to anyone claiming to represent us, as we will never request this information from you.
FORMER CUSTOMERS

We do not share non-public personal information about our former customers with anyone, except as permitted or required by law.

OTHER INFORMATION

This Privacy Policy applies to products and services used primarily for personal, family, or household purposes (non-business purposes).
